We want to thank you for visiting the Westport Plaza Dental Associates website! Our goal is for your first visit to be one of comfort and acknowledgement of your personal dental needs.

Your initial appointment will usually consist of an examination and review of your treatment options. We will take the time to diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan designed to support your optimal dental health.

With your cooperation, we will complete a thorough comprehensive dental evaluation. Because the foundation of a long-term relationship is established at your first visit to our practice, we are committed to providing exceptional service and personalized care from the start.

In many cases, treatment can be completed or started on the same day as your consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment before treatment is provided.

What to Bring to Your First Visit

Please assist us by bringing the following information to your consultation:

• Any x-rays taken by a previous dentist, if applicable.
• A list of medications you are currently taking.
• Any completed dental insurance forms, if applicable. This will help save time and allow us to assist you with processing claims.

Important Information

Patients under 18 years of age must be accompanied by a parent or legal guardian.

Please alert our office before treatment if you have a medical condition that may be of concern, including diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, or similar conditions. Also let us know if you are currently taking any medications, including heart medications, aspirin, or anticoagulant therapy.

X-Rays

If your dentist or physician has taken X-rays, you may request that they forward them to our office. If there is not enough time to do so, please pick them up and bring them with you to your appointment. If additional X-rays are needed, they can be taken at our facility.
